Dental Checkup & Clean package for $129

• Services will only be provided where clinically necessary and appropriate for eligible patients.
• The dental check-up and clean package for $129 includes examination (011, 012), scale and clean (114) or removal of plaque (111), fluoride (121), x-rays (022 x 2).
• X-rays to be taken only when required.
• Offer subject to appointment availability.
• Offer cannot be used in conjunction with any other offer or Government scheme and is not substitutable or redeemable for cash.
• Dental services are provided at Green Apple Dental Clinic by one of our Dental Practitioners.
• The $129 offer is only applicable to those patients who have no private health insurance dental cover and/ or would like to pay it privately.

No Gap Checkup and Clean

• Offer only available to patients who have private health insurance with an Australian health fund who are able to claim their available health fund rebates for dental treatment cover at our clinic.
• Offer includes examination (011, 012), scale and clean (114) or removal of plaque (111), fluoride (121), x-rays (022 x 2).
• X-rays to be taken only when required.
• Health fund limits and rules apply.
• Patient must confirm their eligibility with their private health fund provider before their appointment to claim the rebates.
• If rebates not available, please contact us for our private scheduled fees.
• Your private health fund must at least cover $129 for the full check up and clean with or without x-rays. If the rebate less is than the $ 129, then the patient will need to cover the gap up the $129 privately. If the amount paid by your PHI is more than the $129 we will not be able to pay you back any Gap above the $129 and your offer will be as No Gap dental checkup and clean.
